The Digital Health Passport is designed to be used to support young people with Asthma as part of an annual asthma review. You can send them the app and a copy of their care plan from within EMIS using an Accurx SMS message:
Step 1: Undertake the Asthma Review & save the Asthma Action Plan into the patent record.
Step 2: Using Accurx (or other SMS message service) select the Asthma Action Plan & Digital Health Passport Invitation template in the Asthma section of your practices templates.
